dc.descriptionFunding Data collection for the Dutch interviews was funded by the University of Groningen as part of the MD/PhD program of Malou Luchtenberg. The UK data collection was supported by the National Institute for Health Research. The Academy Ter Meulen Grant provided by the Royal Netherlands Academy of Arts and Sciences enabled the research stay of ML at the Health Services Research Unit, University of Aberdeen, and supported this collaboration. The funders had no role in the design and conduct of the study; collection, management, analysis, and interpretation of the data; preparation, review, or approval of the manuscript; and decision o submit the manuscript for publication.
